Home Modification refers to assistance with modifications and renovations to the homes, gardens and yards of frail older people, younger people with a disability or their carers to help them to cope with a disabling condition and to keep the home in a safe and habitable condition.
What Services are Available?
Modifications
All modification work must be prescribed by an Occupational Therapist. Services provided are:
- Supply & installation of grabrails, handrails, hand held showers and lever taps
- Internal and external ramps, step wedges and steps and paths to suit individual needs
- Widening doorways for wheelchair access
- Bathroom and kitchen modifications
- Safety modifications, eg securing rugs and cords
Home Maintenance
For these services, referrals are accepted from the client or family members, friends, health professionals and other HACC service providers. Examples of the Home Maintenance service:
- Changing light bulbs
- Ensuring areas are well lit to prevent falls
- Repair of unsafe stairs
- Repair of concrete paths to ensure ease of access to the house
- Repairs to guttering above entrances to avoid slip hazards
- Minor electrical & plumbing work
- Lawn mowing service
