Purpose of the Position:
To provide Home Care Packages customers with a phone call to check on their wellbeing.
Provide a monitoring role to people who may be socially isolated.
Main Duties:
Provide a weekly telephone service to recipients of Home Care Packages. Record conversation, feedback and concerns within database.
Demonstrate friendly phone communication to monitor wellbeing and promote social inclusion.
Ensure any concerns are delivered to the Service Manager as soon as possible
